Description

Stone Analysis is a specialized laboratory test used to determine the chemical composition of kidney stones, urinary tract stones, renal calculi, ureteric stones, bladder stones, or passed urinary calculi. This test helps identify the major and minor components of the stone, such as calcium oxalate, uric acid, struvite, cystine, calcium phosphate, or mixed crystal compositions, which is essential for understanding the cause of stone formation, guiding treatment, and helping prevent recurrent kidney stones. Modern reference laboratories commonly use Fourier Transform Infrared Spectroscopy (FTIR) or, in some centers, X-ray diffraction (XRD) because these methods are preferred over older chemical methods for more accurate stone composition analysis. Mayo Clinic Laboratories describes kidney stone analysis using infrared spectroscopy, where representative layers of the calculus are powdered and compared against known reference spectra, while urology literature notes that FTIR or XRD are the preferred analytical procedures for urinary stone composition. Stone analysis is especially useful in patients with first-time stone passage, recurrent nephrolithiasis, family history of stones, pediatric stone disease, metabolic stone disorders, or post-surgical stone retrieval, and results are often used with urine metabolic evaluation, 24-hour urine studies, and urology/nephrology consultation to reduce recurrence risk.

Submit the entire stone / entire passed fragment if possible
Specimen must be clean and dry
Rinse gently with clean / deionized water if contaminated with blood or debris (if lab protocol allows)
Allow the stone to air dry at room temperature
Do NOT use heat to dry the specimen (heat may alter composition)
Place in a clean, dry, sterile screw-cap plastic container
Do NOT send in urine, formalin, saline, or any liquid
Do NOT tape the stone to paper or requisition
