The Free Prostate-Specific Antigen (fPSA) test measures the amount of unbound (free) PSA circulating in the blood. PSA is a protein produced by the prostate gland. Free PSA is usually measured together with Total PSA to calculate the Free PSA/Total PSA ratio (% Free PSA), which helps differentiate prostate cancer from benign conditions such as Benign Prostatic Hyperplasia (BPH) and prostatitis. The test improves the specificity of prostate cancer risk assessment, particularly in men with mildly elevated total PSA levels.
