Leptin (Serum) is a specialized endocrine / metabolic hormone test that measures the level of leptin, a hormone primarily produced by adipose (fat) tissue, in the blood. Leptin plays an important role in regulating appetite, satiety, energy balance, body weight, metabolism, and neuroendocrine function by signaling the brain about the body’s fat stores. This test may be used as an adjunct in the evaluation of obesity, severe early-onset obesity, suspected congenital leptin deficiency, leptin resistance, unexplained metabolic disorders, hypothalamic amenorrhea, lipodystrophy, and selected endocrine or fertility-related conditions. Elevated leptin levels are commonly associated with obesity and leptin resistance, while very low leptin levels may be seen in rare leptin deficiency states, severe undernutrition, or low body fat states. The test is not typically used as a routine screening test and should always be interpreted in correlation with BMI, body fat percentage, insulin resistance markers, glucose metabolism, reproductive status, and clinical findings. Major reference labs list leptin testing as a serum or plasma immunoassay, with fasting recommended (but not always mandatory) and a typical turnaround of 2–6 days.
