Description

HIV 4th Gen (p24 Ag + HIV-1/2 Ab) is an advanced 4th generation HIV screening blood test used to detect HIV-1 p24 antigen along with antibodies to HIV-1 and HIV-2 in serum. This test is widely used for early HIV screening, acute HIV infection detection, routine health checkups, pre-employment screening, pre-surgical screening, antenatal screening, blood donor evaluation, and high-risk exposure assessment. Compared with older antibody-only HIV tests, the 4th generation assay can identify infection earlier because it detects p24 antigen before antibodies are fully developed, improving the ability to detect acute and primary HIV infection. Labcorp states that this test uses a CDC-recommended laboratory diagnosis algorithm for HIV infection, including acute and primary infection, and includes reflex testing when the screening assay is reactive. The assay screens for HIV-1 antibody, HIV-2 antibody, and HIV-1 p24 antigen by enzyme immunoassay (EIA); reactive samples are reflexed to an HIV-1/HIV-2 antibody differentiation assay, and if the screen is positive but differentiation is negative, further reflex testing may proceed to qualitative HIV-1/HIV-2 nucleic acid amplification (NAA/RNA). This test is typically performed on serum and is reported qualitatively as Non-Reactive / Reactive, with confirmatory interpretation if reflex testing is triggered. It is one of the most preferred frontline HIV screening tests in modern clinical practice due to its improved sensitivity during early infection.
