Hepatitis E Virus IgM (HEV IgM) is a serological test used to detect IgM antibodies against Hepatitis E Virus (HEV) in serum, helping in the diagnosis of recent or acute HEV infection. HEV is a cause of acute viral hepatitis and is commonly transmitted by the fecal-oral route, especially through contaminated water or food. The presence of anti-HEV IgM is considered the preferred serologic marker for acute HEV infection and is typically used in patients with jaundice, fever, fatigue, nausea, abdominal pain, elevated liver enzymes, or suspected acute hepatitis, especially in endemic regions or after unsafe food/water exposure. This test is important in the workup of acute hepatitis of unknown cause, travel-related hepatitis, and pregnancy-associated hepatitis, where HEV can be severe. According to the WHO, definitive diagnosis is usually based on detection of specific anti-HEV IgM antibodies in blood, and CDC notes HEV IgM is the serologic marker of choice for diagnosing acute infection.
