HE4 with ROMA is a serum tumor marker profile that measures Human Epididymis Protein 4 (HE4) and CA-125, and uses these values along with menopausal status to calculate the ROMA (Risk of Ovarian Malignancy Algorithm) score. This test is used as an aid in risk stratification of ovarian malignancy in women who present with an adnexal / ovarian / pelvic mass and are being evaluated before surgery or specialist referral. HE4 with ROMA helps classify patients into low-risk or high-risk categories for epithelial ovarian cancer and can support decisions regarding referral to a gynecologic oncologist. The test is not intended as a screening test for ovarian cancer in the general population and should not be used as a standalone diagnostic test. Results must always be interpreted together with clinical findings, ultrasound / imaging studies, menopausal status, and specialist evaluation. Major reference lab catalogs describe the ROMA panel as including HE4, CA-125, and ROMA score (premenopausal and postmenopausal risk calculations) using serum and ECLIA methodology.
