Direct bilirubin is the water-soluble, conjugated form of bilirubin produced in the liver after unconjugated bilirubin binds with glucuronic acid. Measuring direct bilirubin in serum is an essential part of the Liver Function Test (LFT) panel because it reflects how effectively the liver is processing and excreting bilirubin into bile. This test is particularly important when evaluating jaundice, suspected bile duct obstruction, hepatitis, cholestasis, or other forms of liver cell injury.
When the liver’s excretory pathway is impaired or the bile ducts are blocked, conjugated bilirubin accumulates in the bloodstream and may also appear in urine due to its water solubility. By distinguishing direct (conjugated) bilirubin from indirect (unconjugated) bilirubin and total bilirubin, clinicians can better understand whether jaundice is primarily due to hepatic dysfunction or post-hepatic obstruction. This distinction is critical for accurate diagnosis, treatment planning, and monitoring of liver diseases.
