The Calcium / Creatinine Ratio – 24 Hour Urine test is a specialized diagnostic test used to evaluate calcium excretion in relation to creatinine excretion over a full 24-hour urine collection period. This ratio helps standardize calcium output by accounting for urine concentration and kidney filtration, providing a more accurate assessment of calcium metabolism than calcium measurement alone.
This test is commonly recommended for patients with recurrent kidney stones, suspected hypercalciuria, metabolic bone disorders, or abnormal serum calcium levels. Excessive calcium loss in urine is a major risk factor for calcium-based kidney stones and may occur even when blood calcium levels are normal. The calcium-to-creatinine ratio helps distinguish between dietary causes, hormonal imbalances such as parathyroid disorders, and renal causes of calcium loss. It is also useful in evaluating vitamin D–related disorders and monitoring patients receiving treatment for calcium metabolism abnormalities.
