CA 19-9 (Pancreatic Cancer Marker), Body Fluid is a tumor marker test that measures Carbohydrate Antigen 19-9 (CA 19-9) in body fluid specimens such as pleural fluid, peritoneal/ascitic fluid, pancreatic cyst fluid, biliary fluid, hepatic fluid, or other validated body fluids, depending on laboratory policy. This test is used as an adjunctive diagnostic aid in the evaluation of malignancy-associated effusions, pancreatic cystic lesions, and selected gastrointestinal or hepatopancreatobiliary conditions. Elevated CA 19-9 levels in body fluids may support the assessment of pancreatic, biliary, gastrointestinal, ovarian, or other CA 19-9–secreting malignancies, but the test is not specific or diagnostic on its own. It should always be interpreted together with clinical history, imaging, cytology, histopathology, and other fluid markers such as CEA and amylase, especially in pancreatic cyst fluid evaluation. Major reference laboratories list this assay as a quantitative body-fluid tumor marker test performed by electrochemiluminescent or immunoenzymatic immunoassay, with the requirement that the fluid source be clearly specified on the requisition. ARUP’s body-fluid catalog specifically includes biliary/hepatic, CSF, pancreatic, peritoneal/ascites, and pleural fluid; Mayo also offers source-specific validated tests such as pancreatic cyst fluid, pleural fluid, and peritoneal fluid.
