Bence Jones protein (BJP) in urine refers to the presence of free immunoglobulin light chains (kappa or lambda) excreted by the kidneys. These light chains are produced in excess by abnormal plasma cells and are classically associated with plasma cell dyscrasias, most notably Multiple Myeloma. Under normal conditions, light chains are either not produced in excess or are reabsorbed by the kidneys, so they are absent in urine. Their detection in a spot urine sample is therefore a critical diagnostic clue for monoclonal gammopathies.
This test is performed when patients present with symptoms such as unexplained anemia, bone pain, recurrent infections, kidney dysfunction, or elevated total protein. Detecting Bence Jones protein helps clinicians confirm abnormal monoclonal protein production and assess renal involvement caused by light-chain deposition.
