Anti-Parietal Cell Antibody (APCA) is an autoimmune blood test used to detect gastric parietal cell antibodies (IgG) in serum. It is commonly ordered in the evaluation of pernicious anemia, autoimmune gastritis (atrophic gastritis), vitamin B12 deficiency, megaloblastic anemia, chronic gastritis, and suspected autoimmune gastric disorders. APCA targets the gastric parietal cells of the stomach, which are responsible for producing intrinsic factor and gastric acid, both essential for vitamin B12 absorption. A positive APCA result supports the diagnosis of autoimmune gastritis / pernicious anemia, but it is not fully specific and may also be seen in thyroid disease, diabetes mellitus, iron deficiency anemia, vitiligo, and other autoimmune disorders. APCA testing is often performed alongside Intrinsic Factor Antibody, Vitamin B12, CBC, and gastritis workup for better clinical interpretation. Labcorp notes the assay detects gastric parietal cell antibodies of the IgG class in human serum, and their APCA assay is performed by ELISA with a typical turnaround of 2–4 days.
