Angiotensin I is a specialized plasma hormone test used to measure the level of Angiotensin I, an inactive peptide in the renin–angiotensin–aldosterone system (RAAS) that plays a key role in the regulation of blood pressure, sodium balance, fluid balance, and adrenal hormone control. This test is mainly used in the evaluation of hypertension, suspected hyperaldosteronism, secondary aldosteronism, adrenal disorders, renin-angiotensin pathway abnormalities, endocrine hypertension workup, and complex cardiovascular or renal disease assessment. Angiotensin I is formed when renin cleaves angiotensinogen, and it is later converted to Angiotensin II by ACE (angiotensin-converting enzyme). Because this hormone is sensitive to posture, sodium intake, medications, and specimen handling, proper pre-analytical preparation is very important. Mayo Clinic Laboratories lists Angiotensin I, Plasma as a referral test performed on EDTA plasma, using Direct ELISA, with specimen sent frozen, and a reference value of up to 25 pg/mL.
