Acid Phosphatase – Total (ACP Total) is a biochemical test that measures the overall activity of acid phosphatase enzymes present in the blood. Acid phosphatase is a group of enzymes that function optimally in an acidic environment and are found in various tissues of the body, including the prostate gland, liver, spleen, red blood cells, bone marrow, and platelets. Historically, this test was widely used as a tumor marker for prostate cancer before the advent of Prostate-Specific Antigen (PSA) testing, but it still retains clinical relevance in specific diagnostic scenarios.
The total acid phosphatase level reflects contributions from all tissue sources. Elevated ACP levels may be seen in metastatic carcinoma of the prostate, particularly when cancer spreads to bones, as well as in conditions such as hemolytic anemia, liver disease, Gaucher’s disease, thrombocytosis, and bone disorders. It may also increase following prostatic manipulation, catheterization, or digital rectal examination (DRE), which is why proper patient preparation is important before sample collection.
