24 Hour Urinary Citrate (also called Citric Acid [Citrate], 24-Hour Urine or Citrate Excretion, 24 Hour, Urine) is a specialized metabolic stone risk assessment test used to measure the total amount of citrate excreted in urine over 24 hours. Urinary citrate is an important natural inhibitor of kidney stone formation, especially calcium-containing stones, because citrate binds calcium and reduces crystal formation in the urinary tract. This test is commonly used in the evaluation of recurrent kidney stones, calcium urolithiasis, hypocitraturia, renal tubular acidosis (RTA), enteric hyperoxaluria, and for monitoring response to therapy such as potassium citrate supplementation or dietary management. Low urinary citrate (hypocitraturia) is a recognized risk factor for calcium oxalate and calcium phosphate kidney stones. Labcorp notes that urinary citrate excretion is often significantly lower in patients with idiopathic calcium urolithiasis, and that hypocitraturia is common in patients with renal tubular acidosis and enteric hyperoxaluria. Mayo Clinic similarly states the test is useful for diagnosing risk factors in patients with calcium kidney stones and monitoring therapy in patients with calcium stones or renal tubular acidosis. The test is performed on a 24-hour urine collection, and results are usually reported in mg/24 hours. Proper collection is essential because incomplete urine collection or incorrect preservative handling can significantly affect results.
